Docman Summariser - Clinical Coding with Docman

Introduction

This document is a step-by-step guide to the Docman Summariser module.

Docman Summariser allows users to add clinical codes into the clinical system, with attached values where relevant. This module integrates with Intellisense to provide an OCR-based Read Code function to further streamline the read coding process.

Please note – there are two possible clinical coding methods; with or without Intellisense. To clinically code with Intellisense, the site must have the Intellisense product and must have filed the document using Intellisense. This guide covers coding without Intellisense. If you are coding a document with Intellisense, please refer to the Summarising With Intellisense guide.

Workflow and Read Codes

Clinicians have access to a “Read Code Highlighted” action, which will send workflows to the relevant staff member for coding. In this example, Dr Green receives a workflow concerning a patient suffering concussion and loss of consciousness, who has been admitted to emergency surgery. Dr Green uses the highlighter tool to highlight the relevant keywords and uses the “Read Code Highlighted” action to redirect the workflow to the read coding staff.

When Dr Green selects the Read Code Highlighted action, the yellow box in the activity pane changes to display the selected action name and the user or group the action forwards to. Click Finish to move the workflow to the receptionist group for read coding.

The workflow is now sent to the Readcoders group with the action “Read Code Highlighted”, instructing them to clinically code the document.

Accessing Summariser from Workflow

Staff members in the Readcoders group receive the document, with relevant items highlighted by the Doctor. The yellow action box on top of the activity pane lists the requested action. The action also appears in the green History section. To enter the Summariser module, click the Summarise option at the top of the workflow screen.

The first screen that opens depends on how the document was originally filed.

Clinical Coding

The Summarise tab provides details on the read codes that you select, and provides you with the option to manually add read codes. If you do not file the document using Intellisense, the Intellisense tab will not appear and the Summariser will open on this screen by default.

Adding Read Codes

Read codes can be added manually (without Intellisense) using the Actions box in the Summarise tab.

Add Read Code opens the Term Browser. You can use this to search for any Read Code in the system.

This screen also allows you to navigate by the Read Code hierarchy. Highlight a Read Code and press the left arrow key to move backwards in the hierarchy. If the Read Code has a + sign on the left, this means it is not the last branch of the hierarchy and you may highlight it and press the right arrow key to move up to the next level of the hierarchy.

Add Read Code from Non-Values List

Add Read Code from Non-Values List prompts you to select a Read Code from a pre-defined list of codes. This list is maintained in the Maintain Lists section. Left-click a code to select it.

Add Read Code from Values List

Add Read Code from Values List prompts you to select a value-based Read Code. Numerical values can be added to these codes for entry into the Clincial System. A value-based Code may be single-value or multi-value. An example single-value Code is O/E - Height, which prompts for a value of the patient's height. An example multi-value Code is O/E - BP Reading, which prompts for the patient's systolic and diastolic blood pressure readings. To find out how to add and configure value Read Codes, click here.

Left-click a code to select it.

The code appears in the Summarised Items list, along with the single-value codes that form the mutliple values. Add the values under the Selected Summarise Term Details.

Medical History

The patient's previous medical history can be viewed using the Med History tab.

Summariser - Final Steps

The final steps of the clinical coding process take place in the Summarise tab, regardless of how the document was filed.

You are able to remove any Read Code that have been added in error using the Remove From List option on the Activity pane.

Once you have added all necessary Read Code, click Finish to send the information to your clinical system and complete the clinical coding process.

After Summarising

Completing the summarising process returns you to the Workflow screen.

Now that the Read Code are added, click Finish to complete the action and move the workflow on.

The action is now complete, and the workflow moves on from your intray.
